• No of Employees on Last Day of Month: Enter the total number of employees as of the last day of the reporting period.

• Total no. of agents of period and broker (if any): Enter the total number of agents and brokers as of the last day of the reporting period.

• Name of Person who completed this form: Enter the full name of the person responsible for completing the report.

• Designation of Person who completed this form: Enter the job title or designation of the person who completed the report.

• E-mail Address: Enter the email address of the designated person.

• Contact Number: Enter the telephone number of the designated person.
